Other changes** On 9/27/2006, OMB approved this form in two versions. On October 17, 2006, HUD submitted a change request to remove small black boxes in the bottom center of each form. HUD is now requesting that the form with the black border be replaced with this new version. At the request of the industry, HUD is changing from a black border to a gray one, and changing some of the font. This is to make the form easier for lenders to print. There is no change to the text of the document.
