SUBJECT: Extension of the ETA 9048 and ETA 9049 Reports for the

Worker Profiling and Reemployment Services Program


1. Purpose. To inform state workforce agencies (SWAs) that the Office of Management and Budget (OMB) has approved the extension for collection of the ETA-9048, Worker Profiling and Reemployment Services Activity Report and the ETA 9049, Worker Profiling and Reemployment Services Outcomes Report.

3. Background. The Unemployment Insurance Worker Profiling and Reemployment Services (WPRS) program was established to identify, as early as possible in the claims series, those beneficiaries who are likely to exhaust their benefits. The goal is to provide the reemployment services that would be most helpful to the beneficiary as soon as possible. The ETA 9048 report tracks beneficiaries from the time they are profiled and, if selected, referred to services through the completion of services. The ETA 9049 report provides information about their employment outcomes following completion of services that can be discerned from data contained in state unemployment insurance wage records.


4. OMB Approval. This data collection has been approved by the Office of Management and Budget (OMB) under the Paperwork Reduction Act of 1995, OMB approval number 1205-0187, expiration date: XX/XX/XXXX. SWAs are not required to respond to this collection of information unless it displays a valid OMB control number.

5. Disclosure Statement. Based on information obtained from SWAs, it will take an estimated 0.25 hours to prepare, complete and electronically submit to the National Office an ETA-9048 and 0.25 hours to complete and electronically submit an ETA-9049. Respondent’s obligation to

furnish this information is mandatory under Sec. 303 (a) (6) and (j) (1) of the Social Security Act. If electronic transmission of the report is not available, then and only then should the report be sent in by hard copy. In such cases, one copy of the report should be sent to the appropriate Regional Office; the original and one copy should be sent to: U.S. Department of Labor, Employment and Training Administration, OWS/DFAS – Room S-4231, Frances Perkins Building, 200 Constitution Avenue, N.W., Washington, D.C. 20210.


6. Inquiries. All questions or inquires should be referred to the appropriate Regional Office.
