As you may know, Congress directed the National Health Service Corps (NHSC) to evaluate the effects of including [pharmacists/chiropractors] in the NHSC loan repayment program. Mathematica Policy Research, Inc. (MPR) is conducting the evaluation on behalf of the NHSC. As part of the evaluation, they are conducting telephone interviews with all the [pharmacists/chiropractors] who have been serving an NHSC since September 2003. Your participation in the interview is voluntary, but essential to understanding the effects of including [pharmacists/chiropractors] in the NHSC loan repayment program. Your unique insights will help the NHSC and Congress understand the effectiveness of this initiative and how it affects patient access to services and quality of care in underserved areas.

The interview should take approximately 30 minutes. MPR will gather information about the types of services you provided and the contributions you made during your NHSC obligation. They will ask about your role in the clinic’s process of care. In addition, they will discuss your perceptions of the challenges underserved areas experience recruiting and retaining [pharmacists/chiropractors]. Finally, MPR will ask about your satisfaction with your experience participating in the NHSC loan repayment program.

All individual responses to the survey will be kept completely confidential. Your answers will be tabulated and published in aggregate form. However, individual responses may be used on occasion for illustrative purposes. In these instances, the individual [pharmacist/chiropractor] will not be noted.
