Recently, we asked you to complete a survey for a research study on provider practice patterns concerning stillbirth management. The survey packet we sent to you previously included $50 in appreciation for your time. This study is for obstetricians currently practicing in one of five metropolitan Atlanta counties (Clayton, Cobb, Dekalb, Fulton, and Gwinnett). Your answers are very important to us.

Our records show that we have not yet received your completed survey. If you have already completed the survey and sent it in, thank you! If you have not taken and sent in the survey, we would very much appreciate hearing from you at your earliest convenience. We hope that you will help us with this important study.

If you read the first two questions of the survey and decide that you are not eligible, please let us know by returning the enclosed postcard. This will tell us to remove your name from the list of potential respondents. This survey is only for obstetricians currently involved in clinical practice in Clayton, Cobb, Dekalb, Fulton or Gwinnett County.
