Collection Revisions

This submission of the Benefit Application package contains revisions to only the ISBA application. Many of the changes made to the language and instructions are to make the application easier to understand. The additional significant revisions to the application are as follows:

Screen Changes

SSA is making changes to following welcome screens questions (ba002a).

• Delete questions 1 “Do you intend to apply for Social Security benefits over the Internet using electronic signature?”

• Delete question 6 “Have you been convicted of a crime against the United States?”
