The National Survey of Algebra Teachers seeks to obtain information from Algebra I teachers about their views on students’ preparation, curriculum and instruction. You are being asked to participate in this important study. Your participation is entirely voluntary, and all information you provide will be treated as confidential. Failure to provide some or all of the requested information will not in any way adversely affect you. Results from this study will appear in summary form only and will not personally identify you.

The time required to complete this form varies according to individual circumstances, but the average time is estimated to be 25 minutes.
