Custodial Consent Form

Thank you for allowing your child participating in usability testing and/or a focus group session for education-related sections or products of the NASA Web site. This is to inform you that we will be videotaping the session for the benefit of our team members who cannot be present and to enable us to review information in greater detail at a later date.

All elements of this usability test or session will be videotaped with the audio recording. Clips from the video as well as quotes from the session may be used by the team on this project as well as teams on future projects. However, no identifying information will be associated with the clips or quotes.

We will greatly benefit from your child’s feedback and appreciate his or her time. These tapes will not be released outside of the NASA network, and will only be used for reference in our research.

I understand that video and audiotape recordings will be made of the NASA usability testing and/or focus group session. I grant NASA’s usability test group permission to use these recordings for the purposes mentioned above, and waive my right to review or inspect the tapes prior to their dissemination and distribution.
