TERMS OF CLEARANCE: 1. The submitted surveys are approved as individual items under this generic
information collection. Each item under this generic collection will continue to need affirmative approval, via
this online system.
2. Mistakenly, the submission requested an extension of the entire generic information collection. This
approval, however, does not extend the current expiration date. That will be extended only if there is a history
of proper use of this generic collection.
