*The instructions have been revised to reflect the modifications made to the form the modifications are:

• use of Macros;

• explanation of versions of Excel;

• populate worksheet;

• expand worksheet;

• use of “other”; and,

• reporting instructions.

The form: Within the form, immediately below “Project Name”, there is a field for Term, which corresponds to worksheets for Year 1, Year 2 etc. this field is now pre-field.

*Total – for all year(s) of grant. If applicant has 1-year grant the Total worksheet would not be used.

*Goals/Priorities – applicants no longer have to filter back and forth from the NOFA to the Logic Model to determine which of the Department’s Strategic Goals and Policy Priorities will tie into there project the list is provided directly on the Logic Model.

*Reporting – If an applicant is selected for funding, the applicants approved eLogic Model will lock the approved activities/output and proposed projections submitted. Before returning the eLogic Model to the approved applicant it will be unlocked to allow the applicants to do actual reporting.

The following programs modified their logic models drop down tables.

1. Housing Counseling

   • Modifications New/Edited items to Activities and Outcomes

• Need Statements, Measurement Tools, and Management Questions remained unchanged.

2. Continuum of Care

• Added social services and HMIS

3. ROSS-Elderly/Persons with Disabilities

• Added one management question: How many persons improved their living conditions/quality of life?

4. ROSS-Family & Homeownership

• Added two services: Job search and placement, Partnerships with community organizations,

5. Fair Housing Initiative Programs (PEI)

• Changes to reflect the protected classes that were missing and persons hyper-allergenic.

6. Housing Choice Voucher Family Self Sufficiency.

7. Public Housing Family Self-Sufficiency

• Theses two programs made edited to make them more alike.
