The Henry Ford Health System is working with the Centers for Disease Control and Prevention (CDC) to learn more about what women know and think about cancer, including their experiences with cancer in their family, worries they may have about cancer, and their own risk of cancer. CDC is authorized to conduct the study by the Public Health Service Act.

You name was picked by chance from a list of women age 30 and older who receive their health care from Henry Ford Health System to take part in this important research study. The information you and others give us may help us understand how women make decisions about their health. Information we collect also will help CDC better understand who is tested for cancer and why, so doctors can have better ways to talk about cancer testing with their patients.


We are working with a research company, ORC Macro, to carry out the survey. One of our interviewers may call you within the next 30 days. Participation is voluntary. It is up to you to decide if you want to take part in it or not. Whether or not you participate, your care at Henry Ford Health System will not be affected. All information you provide will be kept private and only study staff will be able to see any responses to questions. Your answers to questions will be kept separate from your name or other personal information. Responses from all women without their names or any identifying information will be shared with CDC and HFHS to analyze. We will never use your name in any report. We will always combine your answers with answers from other women in the survey when we write reports of papers.


If you choose to, you may take part in the study before receiving a call from our interviewer by calling this number, «Phone». The phone call is free. You also may call this number to let us know if you prefer that we not call you.
