Thank you for taking part in a recent phone survey for a women’s health research study on ovarian cancer and cancer screening. Enclosed, please find a $15 gift card to thank you for taking part in that survey.

To remind you, the Henry Ford Health System is working with the Centers for Disease Control and Prevention (CDC) and ORC Macro, a health research firm, to conduct this study. You will be called again in about one year to see if you are interested in taking part in a follow-up telephone survey to ask you a few more questions—this call would take no more than 15 minutes of your time. You will receive a reminder letter about one month prior to when we call you to invite you to take part in the follow-up survey. If you complete the follow-up interview you will get a $10 gift card to thank you for your time.

Finally, we have a toll-free number you can call at any time during the next year if you would like to update us with new contact information if you move or change your telephone number. The toll-free number is 1-800-XXX-XXXX. This number is only used for this study; please feel free to leave your contact information on the voicemail if nobody is available to answer the phone when you call. If you prefer, you can simply leave your name and telephone number and someone will call you back to obtain your new contact information.
