Prior Arrests: Record Prior Arrests in the ‘Prior 12-Month Arrest History’ section starting on page 2. Record the ‘Period of Interest’ and the ‘Total # of Arrests in the Prior 12 Months’ in the fields provided. Record prior arrests using one box for each prior arrest that occurred during the period of 1 year PRIOR to the Target Arrest/Incident Date. If more than seven (7) prior arrests occurred, record this additional arrest information by photocopying and completing a blank page 3 for additional prior arrests. Be sure to enter charge codes for ‘Most Serious Charge’ using the charge code list on page 10.
