Revisions to the Detailed Notice

The detailed notice was revised based on public comments received through the 30 public comment period. The title was changed to remove references to “non-coverage” since commenters said that terms like “non-coverage” frighten beneficiaries. The wording of the notice was revised, also based on comment, to more clearly convey that the notice is an individualized explanation of the reasons for their discharge based on medical judgment and Medicare coverage guidelines.
