The Corporation for National Service
Enrollment form is the means by which information is collected on
individuals who enroll for a term of national service in an
AmeriCorps project. The form "reserves" an education award for the
AmeriCorps member and collects demographic information that is
entered into the Corporation's database.

We have re-evaluated the burden
and more members will be enrolled in the program. Also, after a
member complete their portion of the form, the local organization
completes the second portion.
