Dear Organization:
The National Institute for Occupational Safety and Health (NIOSH) of the Centers for Disease Control and Prevention is asking for your help to learn about the workplace (worker) safety and health issues of your members. We would like to know about the workplace (worker) safety and health resources you use or send to your members and any occupational safety and health concerns your members may have.
You will be contacted by the Oak Ridge Institute for Science and Education to complete a short telephone survey. The telephone survey should take about 15 minutes to complete. Participation is voluntary and you are free to skip any questions or stop participating in the survey at any time. The information that you provide in the telephone survey will be used by NIOSH for research purposes and to better meet your occupational safety and health needs. CDC is authorized to collect such information under 9 U.S.C. 669. All information that you and others provide will be handled in a secure manner and will not be released to anyone outside of NIOSH, unless required by law.
