National Survey of the Mining Population

Enclosed you will find a paper questionnaire booklet for the National Survey of the Mining Population. You have the option, however, of completing either this questionnaire booklet or an Internet survey questionnaire. Both versions ask the same questions, although you may find the Internet survey more convenient to complete. For example, the computer will automatically skip any items that are not applicable, based on your response to prior questions.

You can access the Internet survey version by entering the URL www.miningsurvey.org. Once you are at the website, enter the Mine ID Number and the Password that are printed on the label at the top of this instruction sheet to begin the survey. The password is case sensitive, and unique to this particular mine. Then, follow the instructions on the screen.

If you complete the Internet survey, please discard your copy of the paper questionnaire, but save the business-reply envelope, since it may be needed to return supplemental material, as requested in the questionnaire.

The participation of each selected mining operation is vital to the success of the survey, and your cooperation and response is sincerely appreciated.
