Old: 15a. Was patient pregnant/post-partum at the time of first positive culture? 1=Yes, 2=No, 9=Unknown


15b. If yes, outcome of fetus: 1=Survived, no apparent illness, 2=Survived, clinical infection, 3=Live birth/neonatal death, 4=Abortion/stillborn, 5=Induced abortion, 9=Unknown


New: 15a. At time of first positive culture, patient was: 1=Pregnant, 2=Post-partum, 3=Neither, 9=Unknown

“Unknown” was added as an option to the list of Types of Infection Caused by Organism.



Changes made at the request of CDC OMB


  1. Question 11a., second option was changed from “Non-Hispanic or Latino” to “Not Hispanic or Latino”


  1. Question 11.b, third option was changed from “American Indian/Alaskan Native” to “American Indian or Alaska Native”


  1. Question 11.b, fifth option was changed from “Native Hawaiian/Pacific Islander” to “Native Hawaiian or Other Pacific Islander”


  1. Changed “Project Clearance Officer” to “CDC/ATSDR Reports Clearance Officer” in the burden advisory statement on the bottom of the front page


  1. Increased the size of font used in the burden advisory statement on the bottom of the front page
