Changes made by the ABCs program

1. Question 10

   1. Combined “ICU” and “Other Unit” into one “Hospital Inpatient” variable

   2. “Nursing Home”, “Rehabilitation Facility” and “Home Health” taken out and replaced with “Long Term Care Facility”

   3. “Prison/Jail” option deleted

2. Question 11 – All options under “Survived” deleted.

3. Question 16 (on old CRF) deleted

4. Question 17 (old) renumbered as Question 16 (new)

   1. Options for “Bacteremia” collapsed into one variable – “Bacteremia”

   2. Options for “Endocarditis” collapsed into one variable – “Endocarditis”

   3. Options for “Septic Arthritis” collapsed into one variable – “Septic Arthritis”

5. Question 18 (old) renumbered as Question 17 (new)

   1. “Other Drug Use” added in after “IVDU”

   2. “Spider/Insect Bite” deleted

   3. “Decubitis Ulcer” added in after “Immunosuppressive Therapy”

4. Changed “Project Clearance Officer” to “CDC/ATSDR Reports Clearance Officer” in the burden advisory statement on the bottom of the front page

5. Increased the size of font used in the burden advisory statement on the bottom of the front page
