Dear Mr./Ms./Dr. Contact Name:

The Public Policy Research Laboratory (PPRL) at Louisiana State University is working with the Urban Institute, a nonprofit, nonpartisan research organization located in Washington, D.C., to conduct a study of the role that faith-based and community organizations played in providing relief and recovery services after hurricanes Katrina and Rita. The study is sponsored by the U.S. Department of Health and Human Services, and the results will help the government and major relief agencies plan for ways to better work with faith-based and community organizations after a disaster.

We will be calling you in the next few weeks to conduct a short telephone survey to find out if you provided relief services after the hurricanes and your experiences in doing so. If you offered services, we will be asking questions about what types of services you provided, and whether you worked with other organizations in providing services.

The survey will take about 20 minutes. Please be assured that your answers will be treated as a private matter, within the limits of the law, and your responses combined with others’ to maintain anonymity. Your name will never be used in any report and your answers will be combined with answers from other groups that provided similar services and reported in summarized form only.

Your participation in the survey is voluntary, but very much appreciated. The results of this study will help our nation and the region be better prepared to respond to emergency and disaster situations. We would very much like to hear your experiences and hope you will participate in this important study.

For more information about the study, please call Steve Procopio at 225-578-7499 (email: [email protected]) or Carol DeVita at 202-261-5232 (email: [email protected]).

Sincerely,

Steve Procopio Carol J. De Vita Fredrica Kramer
