Block 6:

Request you add a blank for a phone number.

Justification - we often receive the form from an attorney who certifies the document in block 6, but who represents the applicant in block 1. As a result, the phone no. in block 6 is different from the phone no. in block 1 - but we don't have it available.

We're different from DFAS in that the Coast Guard is much smaller - which means we sometimes actually do call the certifier :-)
