This ICR is approved consistent with changes made to the revised UDS manual, in response to public comments. Most importantly, HRSA agrees to collect data on lead screening through other instruments and to ensure that patients who are sampled for clinical outcome measures can be reasonably expected to rely on CHCs as their usual source of care (e.g. by including only those patients who have had at least 2 encounters with a CHC professional within the reporting period for the diabetes measure). Nevertheless, HRSA shall exercise caution when interpreting results and provide all appropriate disclaimers about potential data limitations. Finally, multi-site grantees shall ensure that the patients they sample for all UDS measures are representative of their patient caseload, rather than selecting patients from only one particular site.

The Uniform Data System (UDS) is the Bureau of Primary Health Care's annual reporting system for grantees. The UDS includes reporting requirements for grantees of the following programs: the Community Health Center program, the Migrant Health Center program, the Health Care for the Homeless program, and other 330 funded grantees. The UDS is the tool that is used for monitoring and evaluating health center performance, and for ensuring compliance with legislative mandates.
