The application/certification forms
(referred to as MT-1 series forms) are designed to assist
requestors in gathering information that FEMA needs to determine
whether a certain property is likely to be flooded during a flood
event that has a 1 percent change of being equaled or exceeded in
any given year (base flood).
An program was made to this
information collection to capture the correct purchasing of service
cost to respondents. The total number of respondents included for
this cost should have been 19,972. The total cost should be
8,987,400. (19,972x $450) vs. 8,222,000. An increase of +$765,400
was included in purchasing of service cost to respondents for this
collection. The total number of responses for this information
collection is 41,633 vs. 39,633. This additional response include
1,700 responses for the On-Line Tutorial.
