1. Complete the above information for each set of samples sent to the lab.
2. Collect an ear notch from each calf born between November 1, 2007, and July 15, 2008.
Place each sample in a separate whirlpak bag, express air, and twist down twice.
3. IT IS CRITICAL that all data on the label are completed with a permanent pen
(Sharpie®) and affixed correctly. Getting the correct result back for each calf will be
important for future management decisions.
4. Freeze the samples as soon as possible and for at least 24 hours. Freeze icepacks for at
least 24 hours before shipping.
5. To ship, place samples in the larger bag and secure with rubber band, twist tie, or knot.
Place 2 ice packs in the bottom of the supplied shipping box, the samples, and then 2 more
ice packs.
6. Place the YELLOW copy of this submission form between the inner and outside flaps of the
box. Mail the ORIGINAL to the NAHMS program using the supplied Business Reply
envelope. Keep the PINK copy for your records.
7. Contact FedEx for the nearest location that accepts EXPRESS packages and note the last
drop-off time. Ship on Monday, Tuesday, or Wednesday.

According to the Paperwork Reduction Act of 1995, no persons are required to respond to a collection of information unless is
0579-NEW. The time required to complete this information collection is estimated to average 6 hours per response, including
the time to review instructions, search existing data resources, gather the data needed, and complete and review the
information collected.
