The U.S. Department of Agriculture’s Animal and Plant Health Inspection Service (APHIS), the State of
and the Producer hereby enter into this National Animal Health Monitoring System (NAHMS) Beef 2007-08 study
PRODUCER AGREEMENT, the terms of which are set forth below.
APHIS and/or the State of
will provide personnel who will be referred to as the Data Collector.
The Data Collector and the Producer will participate together in implementing a statistically valid NAI-IMS study for
determining national estimates of beef-health practices and for compiling health information to enhance beef production.
The Data Collector will complete two person-to-person interviews with the Producer.

The Producer will assist APHIS by providing accurate infumaation regarding beef-health and management practices
related to the study objectives. The Producer retains the right to refuse any questions deemed inappropriate.
The Data Collector will keep the origin of the data confidential by recording the data with the Producer’s unique code
number only. The Data Collector will not keep any key to the code aider the completion of the study. The Data Collector
and all other project personnel acknowledge that the Producer is providing information that he/she does not customarily
share and is providing it with the expectation that it will not be made public. The one exception to data confidentiality is
the suspicion or diagnosis of a dangerously contagious, infectious, or exotic disease foreign to the U.S. on the Producer’s
premises (e.g., fuot-and-mouth disease), in which case further investigation and possible action may occur.
Data collected by the Data Collector will not be used for regulatotypurposes. However, information on a Producer’s
animals revealed from sources unrelated to the Beef 2007-08 study, such as testing and inspection for movement or sale of
animals or tracebacks on testing done at slaughter, may cause regulatory action to be initiated by the State or APHIS.
APHIS may publish, or authorize others to publish, the aggregate (summary) findings acquired from NAHMS for the
benefit of the beef industry, private industry, and other interested groups, but will ensure that the identity of the Producer is
withheld. APHIS may not publish, or authorize others to publish, individual responses.
After completion of data reporting by the Producer, APHIS will provide the Producer with several reports containing
summary results from all participating Producers. The Producer can obtain any further information available from this
study by accessing the NAH~S Web site or subscribing to the NAHMS beef mailing list.

The Producer will complete a brief evaluation of the Beef 2007-08 study, the results of which will be used to assist APHIS
in the design and implementation of future NAHMS surveys.
Any changes to or waivers of the terms of this PRODUCER AGREEMENT shall be binding on APHIS and the STATE of
and the Producer only if they are put in writing by each party.

(a) The Producer consents and authorizes the Data Collector to
collect 30-40 samples of cow fecal material from the ground.
All samples will be tested for Salmonella, and the results returned
to the Producer as positive or negative within 45-60 days. A subset
of samples will be tested for various pathogens including, but not
limited to, Campylobacter, Enterococcus, and Clostridium difficile.
These results will not be returned to you.
Information about Items (b) and (c) is described at the bottom of this page.
(b) The Producer reviewed the information regarding the BVD
testing of ear notches from calves and would like his/her herd
to participate in the sampling. Producers selected to participate
will be notified by the Data Collector within XX days.
(c) The Producer reviewed the information regarding the
parasite testing of fecal samples and would like his/her herd
to participate in the sampling. Producers selected to participate
will be notified by the Data Collector within XX days.

Additional information for Items (b) and (c):
Two additional biologic collections available to qualifying Producers."
¯ Ear notch taken from calves and tested for BVD virus (persistent infection)
¯ Manure tested for parasite burden, and possibly dewormer resistance.
Both require the Producer to collect and ship the samples following specific protocols and timelines. Collection
materials and shipping costs are provided.
The Producer will also need to allow his/her contact information (name, address, phone number, and/or
e-mail) to be available to a member of the NAHMS staff in Fort Collins, CO, in order to directly send the collection
materials and results.
The Producer will need to get the samples to a local Federal Express location that accepts Express packages.
A separate Producer Agreement will need to be signed to participate in the BVD and parasite sampling.
(b) BVD persistent infection - for spring calving herds only
An ear notch is taken from all calves born between November 1, 2007, and July 15, 2008. The Producer will need to
record the CatfID and age for each calf and to keep samples frozen until shipped. Results will be returned quickly
so herd-management decisions can be made based on the outcome.
(c) Parasite burden - for replacement heifers
Level 1 - The Producer will provide answers to about 18 questions and collect 20 fresh fecal samples off the ground
from a group of replacement heifers. The samples will be collected via a set protocol and timeline and shipped
within 24 hours of collection via Federal Express.
Level 2 - Producers who deworm their replacement heifers and would like to evaluate dewormer effectiveness can
participate in Level 2. Replacement heifers will be dewormed within a few days of the Level 1 collection and
another set of fecal samples collected within 10-14 days of deworming.

Level 1 - Evaluate parasite egg count

By signing this Agreement, the Producer is allowing his/her name, shipping and mailing address, phone number, and e-mail
address to be given to a member of the NAHMS staff for the purpose of sending the collection materials (kit) and testing
results directly to the Producer and to contact the Producer ifa problem arises with the samples. NAHMS will maintain this
information in a confidential manner, use only for the purposes described, and will destroy the data when the study is over.

The Producer will complete the submission paperwork including answering the survey questions. Producer can opt out of
questions he/she finds objectionable.
The Producer will collect the fecal samples offthe ground from a pen of replacment heifers. The Producer will follow the
stated collection and shipping protocols included with the kit. This includes taking 20 golf-ball size samples, each from a
separate FRESH manure pat, writing the Producer ID and Kit Number on the label and placing on the bag; shipping samples
within 24 hours of collection via Federal Express; and shipping overnight on a Monday, Tuesday, or Wednesday.
5. NAHMS will return results directly to the Producer within 4-6 weeks.

ability to test the samples may be compromised and the samples may not be testable.

Level 2 - Evaluate dewormer effectiveness
By signing below, the Producer agrees to participate in Level 2 which requires the following:
1. Within 24 hours of the Level 1 collection, the Producer will deworm the Replacement Heifers in the group sampled.
2. Within 10-14 days of deworming; the Producer will collect 20 samples in the same manner as Level 1 and from the same
Replacement Heifer group.
3. NAHMS will return results directly to the Producer within 4-6 weeks.

address to be given to a member of the NAHMS staff for the purpose of sending the collection materials (kit) and testing
results directly to the Producer and to contact the Producer ira problem arises with the samples. NAHMS will maintain this
information in a confidential manner, use only for the purposes described, and will destroy the data when the study is over.

The Producer’s herd must be primarily a spring calving herd. The Producer will collect one ear notch from each calf born
between November t, 2007, and July 15, 2008.
The Producer will complete the submission paperwork and submit copies as instructed to the NAHMS staff and to the
laboratory with the samples. The Producer will follow the stated collection and shipping protocols included with the kit. This
includes placing the ear notch in an individual bag; labeling each bag with the Producer ID, calf[D, and age; freezing the
samples; and shipping overnight via Federal Express on a Monday, Tuesday, or Wednesday.
Kits containing the collection materials, including the ear notcher, will be provided by the study program. Kits will be
delivered directly to the Producer at approximately the dates determined in the following matrix. Working through the
following table, the Producer and Data Collector will establish approximate delivery dates for kits. The Producer will notify
the Data Collector if anything occurs that will affect the established delivery date(s) of kit(s) (e:g., decide to process calves
sooner than expected).
