Appendix E.2 Enrollment Packet Cover Sheet

Yellow Folder Instructions – Materials to Keep

Thank you for your interest in the Study to Explore Early Development (SEED). All of the materials in the Yellow Folder are for you to keep for your records. Please look for the following items in the Yellow Folder:

1. Study Flow Chart: The flow chart gives an overview of the study. Please use the chart to identify what steps you and your child have completed. You can also use the chart to see what the next activity will be.

2. Consent Form: The consent form gives you all of the information about you and your child’s participation in this study. This copy is for you to keep. When you come to the clinic for the first time, study staff will review this form with you, answer your questions, and have you sign a copy of the form. You will be given a copy of the signed consent form to keep. The copy in this folder is for you to review so that you have a chance to think about questions you may have.

3. Biological Sampling FAQ: This document answers frequently asked questions (FAQ) about biologic samples.

4. Participant Preparatory Guide and Scheduling Booklet: The prep guide and scheduling book contains information that will help you prepare for the study. It also contains a scheduling grid to help you keep track of your appointments.

5. Social Story: The social story will help you and your child prepare for the clinic visit. It includes pictures and descriptions of each event in which your child will participate. Please read it to your child before you come for the clinic visit.

When you are finished with this folder, please complete and return the materials in the Green and Blue Folders.
