On September 4, 2007, the Medical IRB C deferred final approval of a new protocol to an IRB Chair/Designee. Final approval will not be granted until an IRB Chair/Designee reviews and approves the revisions requested below. The following is a summary of the Board¹s requests:
· Provide a HIPAA Compliant Form or Attachment 46 when obtaining
medical records from health care providers outside of our medical center.
