Revision to the Collection Instrument

SSA is requesting formatting changes to the form that will allow faster processing of the SSA-1588 which will lead to better customer service.

The requested changes are as follows:

• The "change of address" information will be moved from the envelope to the form

  • The Address Area (for corrected address) will be enlarged to accommodate a change in address

  • A check box will be added to indicate a change in address

• The boxes for Year in 1.B will be expanded to four positions (e.g., “2004” instead of “04”)

• The perjury statement will be added.

The above changes are being made to facilitate faster electronic processing of the form which will lead to benefits being paid quicker to the beneficiaries.

Upon approval of these changes, SSA will obsolete the previous versions of the form and destroy existing stock so as not to confuse beneficiaries by mailing out different forms.
