On the application in Attachment 1, the following question was added just below the project description: “Does the applicant use an electronic health record (HER) that captures, maintains and manages patient health information?”


To identify nursing homes that are implementing health information technology

On the payroll submission guidelines in Attachment 2, under Data Elements, the definition of “Certified Nurse Aide” has been clarified. Medication Aides and Restorative Aides must be State certified in order to be included in the Nurse Aide category. Also, hours worked by nurse aides in training will not be included in this measure. On the data collection form on attachment 3, the instructions in Section C on nurse aides have been changed so that nurse aides in training are not included.

To remove inconsistencies in and clarify the definition of the nurse aides category for payroll submission.
