The National Recreational Boating Survey
This survey is being conducted to find out about people’s recreational boating experiences and to
improve the effectiveness of boating safety programs. It is being administered by [contractor name] on behalf
of the U. S. Coast Guard, in collaboration with the National Association of State Boating Law Administrators
(NASBLA) and your state boating officials. By responding to this survey, you are playing an important role in
helping the U.S. Coast Guard improve boating safety and the quality of boating experiences across the country.

♦

Using boat registration files, we have sampled one of your registered boats. When responding to
questions referring to your “identified boat” please answer about the boat noted in the box below.
This questionnaire is designed to be completed by the recreational boating participant with the most
recent birthday. If the selected boating participant is a child under the age of 16, then the
questionnaire should be completed with the assistance of an adult.
Boat-specific questions (#11 through #22 and 25 through 44) are to be answered with the assistance
of the owner of this registered boat or the person in the household most familiar with the sampled
boat.
Your “identified boat”: [description of boat]

If you no longer own this particular boat, please continue anyway. The questionnaire will address this
situation.

Your responses to this survey will be kept completely confidential and will only be reported in combination
with the responses of others. Your privacy will be protected to the maximum extent allowable by law, and your
name will not be given to the U.S. Coast Guard or any other organizations.
Please record your answers with either a pen or pencil. When you are finished with the survey, please place it
in the enclosed self-addressed, postage-paid envelope and return it to [contractor name] within two weeks of
receipt. If you have any questions please contact [contractor contact person] at [telephone number] or [e-mail
address].

Thank you very much for taking the time to complete this survey.
An agency may not conduct or sponsor, and a person is not required to respond to a collection of information unless it displays a valid OMB
control number.
The U.S. Coast Guard estimates that the average burden for this report is 20-25 minutes. You may submit any comments concerning the
accuracy of this burden estimate or any suggestions for reducing the burden to: Commandant (G-3PCB-1), U.S. Coast Guard, Washington,
DC 20593-0001.

*Registered Boats: Most states require that boats with motors or engines be registered in the state where
the owner resides or the boat is operated regardless of their length. Many states also require boats over a
certain length to be registered even if they do not have a motor or engine. The size requirement varies from
state-to-state. Some boats can be registered and also U.S. Coast Guard documented.
**U.S. Coast Guard Documented Boats: Some vessels that measure at least five net tons and are wholly
owned by a citizen of the U.S. can be documented. Most vessels more than 25 feet in length will measure
five net tons or more. Vessel documentation is a national form of registration that provides conclusive
evidence of nationality for international purposes. Many financial institutions require vessels which are
eligible for documentation to be documented and to have preferred mortgages recorded against them.
