You are being asked to participate in a research study entitled, “Evaluation of the Safe Dates Project.” You have been selected to participate in the implementation evaluation because of your involvement as a principal in a school receiving the Safe Dates curriculum. This study is funded by the Centers for Disease Control and Prevention (CDC) and is being conducted by RTI International, a not-for-profit research organization. The purpose of the implementation evaluation is to help us understand implementation of violence prevention activities at your school.


In you agree to fill out our Web-based survey, we ask that you please answer the questions honestly and in a private setting. You may access the survey by going to this website [online address to be inserted].

The purpose of the implementation evaluation is to help us understand what type of activities occurred as part of the Safe Dates program, including how the program was delivered. The purpose of the cost evaluation is to help us understand the economic and time costs of delivering the Safe Dates program.
