Appendix H. Letter to Parents to Recruit Students for Effectiveness Evaluation

Dear PARENT NAME:


Your child’s school is taking part in a research study about youth violence, including violence between preteen and teen dating couples. The study is sponsored by the Centers for Disease Control and Prevention (CDC) and will be conducted by RTI International, a not-for-profit research organization in Durham, North Carolina. Your child was selected for the study because of the school and grade your child is in.


As part of the study, your child’s classroom may be observed to learn about the teacher’s violence prevention curriculum delivery, the number of students in the class, and what was covered. The observer will not record any names or other personal information about your individual child and will not ask your child any questions or speak during the class time.


The attached consent form asks your permission for your child to participate in this study by completing up to four surveys.
