Evaluation of the Safe Dates Project


BEGIN


Hi. I am (FIRST AND LAST NAME OF RTI or HEADWAY TRACER) from RTI International.


Last year, you gave permission for (YOUTH FIRST) to complete a survey about youth violence, including violence between preteen and teen dating couples. This survey is conducted by RTI International, a not-for-profit research organization in Durham, North Carolina, and is sponsored by the Centers for Disease Control and Prevention.


We would like to schedule a time to give the follow-up survey to (YOUTH FIRST). We would like to interview (YOUTH FIRST) again, to see whether his/her experiences, attitudes and opinions about these topics have changed over time. It should take about 50 minutes for (YOUTH FIRST) to complete the survey.


The follow-up survey will again ask questions about how your teen feels about dating, communicating with a dating partner, and attitudes and behaviors related to violence, including violence between preteen and teen dating couples, and what he/she is learning in school about how to prevent and avoid violence. Some questions will ask about dating violence that may have happened to him/her or that he/she may have done to someone else and how he/she felt about it. Your teen does not have to have dated, had any violence happen to him/her, or done anything violent to someone else in order to complete this survey.


After (YOUTH FIRST) completes the survey, we will give him/her a $25 gift card as a token of our appreciation for his/her participation in this survey.


If this all sounds OK to you, we can go ahead and schedule a time and place for (YOUTH FIRST) to take the survey. If you or he/she prefer, we can complete the follow-up survey over the telephone.
