The Agency for Healthcare Research and Quality (AHRQ) hereby requests re-approval of a previously approved generic clearance titled “Questionnaire and Data Collection Testing, Evaluation, and Research for the AHRQ 0935-0124."
Attached are the supporting statement and other supporting documents for the requested clearance. Per your email dated 1/15/2007, also attached is a clearance package for the proposed information collection titled “CAHPS Pretest of Proposed Health Literacy Questions and Methodology 0935-0124,” submitted under the generic clearance we are seeking reauthorization for.
These data collection activities are not required by regulation and will not be used by AHRQ to regulate or sanction its customers. Participation by the public is entirely voluntary and confidentiality of respondents and their responses will be preserved.
