The OSHA-7 Form is used by OSHA
personnel to report unhealthful and/or unsafe conditions in the
workplace. The information is given to OSHA by employees who wish
to report unhealthful and/or unsafe conditions at their place of
employment. Employee reports are authorized by Section 8(f)(1) of
the OSH Act. This information is used by OSHA to evaluate the
alleged hazards and to schedule an inspection.

The burden hour reduction, from
13,611 hour to 12,775 hours is primarily due to a decrease in the
estimated total number of OSHA-7 forms being processed (forms can
be sent electronically, orally or mailed) from 50,995 to 48,298.
The approved ICR estimates the cost burden at $692. This was
rounded to $1,000 on the OMB 83-I, and entered into ROCIS. As a
result of the increase in mailing costs and the number of forms
being mailed, OSHA estimates a total cost of $990; an increase of
$298 from the previous $692. However as a result of rounding, ROCIS
reports a reduction of $10.00.
