FCC Form 395

The FCC has made a minor change to FCC Form 395, “Common Carrier Annual Employment Report” contained this information collection 3060-0076.

The minor change removes the “Expires: xx/xx/xx” line from the upper right corner on the first page of this form.

The Commission made this change pursuant to its response to Question 17 of the Supporting Statement in which we stated:

“The Commission does not intend to seek approval not to display the expiration date for OMB approval of the information collection; instead, the Commission will use the edition date in lieu of an OMB expiration date on these forms.”

The edition date for FCC Form 395, “Updated December 2007,” is located in the lower right corner on the first page of this form.
