The information called for in this questionnaire is for use by the United States International Trade Commission in connection with its
investigations concerning the use of subject denim fabric (Inv. No. AGOA-002) and the availability of subject denim fabric (Inv. No.
AGOA-003) under amendments to section 112(c) of the African Growth and Opportunity Act (19 U.S.C. 3721(c), included in Public
Law 109-432. The commercial and financial data furnished in response to this questionnaire that reveal the individual operations of
your firm will be treated as confidential by the Commission to the extent that such data are not otherwise available to the public and
will not be disclosed except as described in the certification paragraph below, and as may be required by law. This report is
mandatory and failure to reply as directed can result in a subpoena or other order to compel the submission of records or
information in your possession (19 U.S.C. ' 1333(a)).

The questions in this questionnaire have been reviewed with market participants to ensure that issues of
concern are adequately addressed and that data requests are sufficient, meaningful, and as limited as
possible. Public reporting burden for this questionnaire is estimated to average 4 hours per response,

Note: Denim fabric classified in subheading 5211.42.00 of the HTS is made from the same types of yarns and weaves
as that of the subject denim fabric. However, denim fabric classified in subheading 5211.42.00 contains less than 85
percent by weight of cotton and is made of both cotton and manmade fiber yarns.
