In November of 2003 the Diabetes Detection Initiative (DDI) implementation was launched. The DDI was a national pilot to mobilize community-based efforts targeted toward identifying a portion of the estimated 5 million people with undiagnosed Type 2 diabetes, refer them for diagnostic testing and, if appropriate, follow-up treatment. The Centers for Disease Control and Prevention (CDC) has asked Battelle to obtain information regarding the costs associated with your clinic’s current activities in the areas of diabetes awareness and screening, as well as the number of patients screened or diagnosed. We will be mailing you the survey within the next two weeks.
