1. The OMB number is missing from the form and will be included in the upper right hand corner of the first page of the form.

Page 1, Introductory Paragraph

1. Insert “under 8 C.F.R. § 1003.15(d)(2)” after “You are required” and before “to notify”.

2. Insert “working” after “five (5)” and before “days of moving”.

Grey box

1. The first bullet point in the grey box. Add “Department of Homeland Security (DHS)” and strike out “DHS”.

2. Below the grey box, insert a bolded box around “Name: ______” and “Alien Number: A ____”.

Proof of Service

1. Change “Assistant Chief Counsel” to “Office of the Chief Counsel”.

2. Move “You Must Complete This” to the same line as “PROOF OF SERVICE.”

3. Lengthen blank line with annotation “(Number and Street, City, State, Zip Code)”.

Page 2, Mailing Instructions

1. Add as Mailing Instruction Number 1: “1. Copy the completed form and mail or deliver it to the Office of the Chief Counsel DHS-ICE address you inserted in the PROOF OF SERVICE. The PROOF OF SERVICE certifies that you provided a copy of the form to DHS.” Re-number the “Mailing Instructions” accordingly.

2. Edit Mailing Instruction Number 5 (now Number 6) by adding “original” in between “the” and “form”. Also add “to the Immigration Court whose address is printed below” before “form”.
