Dear Chief:
As you well know, fire departments play a vital role in educating the public about fire and life safety. Yet, we have no clear picture of what is being done in the state of Pennsylvania or how it’s being done. The National Center for Injury Prevention and Control at the Centers for Disease Control and Prevention (CDC) asked Johns Hopkins University to survey fire departments to find out. Please help us by completing the enclosed survey. The results will be used to highlight successes and find new ways to address the challenges of fire and life safety education (FLSE).
The questions ask about your department’s FLSE resources, activities, materials, and training needs. All recognized fire departments in Pennsylvania are being asked to complete this survey. The survey can be completed in 20 minutes or less. Please know that your participation is completely voluntary. Your department has been assigned a unique identification number (located in the top right corner of your survey) to keep your answers confidential. Your responses will not be linked to your name or the identity of your fire department. If you do not want to be contacted again about this survey, you can call 410-614-4027 or send an email to [email protected].
We know you are very busy and we greatly appreciate your taking the time to help us with this survey.
Sincerely,
If you are not the fire chief and received this letter in error, please contact Jennifer Piver ([email protected]). If you have questions about the survey or its results, please contact Dr. Andrea C. Gielen, the Principal Investigator at Johns Hopkins University at 410-955-2397.
