Description of Changes to Revised (FY2008-FY2010) Questionnaires



The Indicators for School Health Programs are Web-based questionnaires corresponding to the specific funding priorities from DASH. There are three questionnaires for FY2007 data collection. Two pertain to HIV-prevention program activities among LEAs and SEAs/TEAs, and the third pertains to CSHP/PANT activities among SEAs. These three questionnaires had previously received OMB approval.

Four questionnaires will be used for FY2008-FY2010 data collection. Three questionnaires are revised versions of the HIV-LEA, HIV-SEA/TEA, and CSHP/PANT questionnaires. These received only minor updates to provide more user-friendly response sets and to bring the survey in line with the requirements of the new Funding Opportunity Announcement that will support program activities beginning in FY2008. In addition, some questions were rearranged to be presented in the more logical order, and multiple choice options were added to eliminate the need for some open-ended answers.

The fourth questionnaire for FY2008-FY2010 is a new questionnaire designed to track activities of asthma management programs conducted by LEAs. The asthma management questionnaire was added in response to an increase in the number of asthma management programs receiving funds from DASH in the new Funding Opportunity Announcement (DP08-801). The questionnaire’s question structure and format were based on the previously used HIV prevention and CSHP/PANT questionnaires, but the content was tailored to the specific characteristics of asthma management programs. The most noticeable difference is the addition of a section for health-related services. Feedback from CDC/DASH staff was instrumental in creating and refining the questionnaire.
