New or Revised Data to be collected beginning January 1, 2008 for the January-June 2008 report, to be submitted by July 30, 2008


  1. GENERAL INFORMATION


A1. Type of implementing agency/organization

We will be adding a category for Sexual Assault Programs

Is that organization/faith-based?

Grantees will be asked whether the lead organization is a faith-based organization.



A2. Staff information

New categories of funded staff have been added:

  • Trainer

  • Translator/interpreter

  • Victim advocate

C1. People trained

The following new categories have been added:

  • Guardians ad litem

  • Translators/interpreters


The following training content areas have been added:

  • Child development

  • Dating violence overview, dynamics and services

  • Issues specific to victims/survivors who have limited English proficiency


C2. Coordinated Community Response

There will be some additional categories of agencies/organizations:

  • Substance abuse providers

  • Sex offender management/sex offender treatment

  • Educational organizations/institutions

You will be asked to report on the number of communities with improved capacity for a coordinated community response to domestic violence, dating violence, sexual assault and stalking.

F. NARRATIVE


Optional focused narrative questions

At the end of most of the sections of the form there will be an optional narrative question that gives subgrantees the opportunity to provide additional information regarding that particular activity area. Following is an example from the training section:

Use the space below to discuss the effectiveness of your training activities and to provide any additional information you would like to share about your training activities beyond what you have provided in the data above. Examples might include a reduction in inappropriate referrals following a judicial training; improved system response to victims with disabilities following a multidisciplinary training specific to victims with disabilities.

The following sections of the forms have this question: training and staff development, CCR, policies, planning, and services.


Optional data clarification question

A final question allows subgrantees to provide additional information to explain and clarify the data contained in the reporting form.
