On page 3, Section D, question 1, we plan to change the last sentence from

“If yes, give the date the educational institution implemented the IEP and proceed to next question.  Date IEP implemented:”

to

 

“If yes, give the date the educational institution initially implemented the IEP and proceed to next question.  Date initial IEP implemented: __________      Date current IEP implemented (if applicable): __________”

Rationale for Change:

This change makes it easier to document if students have more than one IEP.
