Abstract: VA Form 21-4176 is used to gather information that is necessary to determine eligibility for compensation or pension benefits based on disability which is the result of an accident. Benefits are not payable where an injury is the result of willful misconduct. Without this information, VA would be unable to properly authorize benefits.
