Instructions for CCC-752

2008 SUPPLEMENTAL AGRICULTURAL DISASTER ASSISTANCE BUY-IN FOR INSURABLE CROPS

CCC-752 allows producers that did not purchase crop insurance coverage for 2008, to be eligible for 2008 Supplemental Agricultural Assistance program payments. Producers pay a buy-in fee of $100 per crop per county; or $300 per producer per county; not to exceed a total of $900 per producer. The buy-in fee is non-refundable and due at the time a producer files form CCC-752 at their county office. The buy-in fee waives the crop insurance or NAP coverage requirement. It does not provide coverage for the 2008 crop year.

Producers submit an original, signed and dated copy of CCC-752 to their county FSA office when applying for 2008 Supplemental Agricultural Assistance program payments. The deadline for submitting CCC-752 will be determined. Please contact your county office.

Items 1- 12 are for County Office use only.
