Justification for Nonsubstantive Change Request

This nonsubstantive change request transmits the final survey instrument. Cognitive testing resulted in a few relatively minor question wording changes. The only noteworthy change was the answer format for Question G series. We changed the rank ordering of programs and services to a more simplified importance Likert scale. The task of rank ordering proved too difficult to administer over the phone.
