NATIONAL INSTITUTE FOR OCCUPATIONAL SAFETY AND HEALTH (NIOSH)

SCHOOL SURVEY

MONTH, YEAR

We (NIOSH) are partnering with the American Lung Association of New England to improve indoor environments of schools. We will be doing a three-year research study that includes both health and environmental surveys of the school. This year the surveys will be during the week of DATE.

We are inviting all school employees to participate in a questionnaire and lung function testing. In each of the three years you will asked to take part in the study. Participation is entirely voluntary. For the study to be as successful as possible, it is best for all employees to take part in all three years. The questionnaire will be given by an interviewer and will take about 30 to 45 minutes. The lung function testing will be done by trained NIOSH technicians and will take about 10-15 minutes. The questionnaire and lung function testing will be done at the school during school hours.

Note: The paragraph below will only be included on the flyer sent to the school with dampness problems.

Some participants will be asked to take part in a lung function test to look at changes in lung function over time. This will occur during three weeks in the spring semester and three weeks during the summer.

All results will be confidential and will become part of the CDC Privacy Act System. Overall study results (without names or other identifying information) will be prepared and will be available upon request.
