Dear [NAME],
About one year ago you joined a national study called the Youth Transition Demonstration
Project (YTDP). The study is sponsored by the Social Security Administration. You were eligible
for the study because you received benefits from Social Security. The YTDP is testing ways to help
young people with disabilities become more independent. More than 5,000 young people are
included in the study. When you joined the study you agreed that we could call you in one year to
ask some questions about you are doing. I am writing you today about that call.
An interviewer from Mathematica will call you in about a week. The interviewer will ask
questions about your school, work or job training experiences, and how you get along day to day.
You can have someone help you answer the questions. This survey is voluntary but very important.
It will help us learn how the programs offered by Social Security affect your life. We would like to
talk with you even if you no longer receive benefits from Social Security. As a token of our
appreciation, we will send you a $10 Target gift card for participating in the interview.
If your telephone number has changed since last year, it is especially important that you call us,
because we will not be able to call you. If you have any questions, or wish to set up a time for the
interview, please call us at 800-298-3383. There is no cost to call this number. We will call you in
a week if we do not hear from you. You may also e-mail MPR at [email protected]. If
you are deaf or hard of hearing, call 877-542-6734 (TTY) to talk to the research staff.
