1. Enter name of Principal Advisor.
2. Enter date and time of State request.
3. Enter date and time of Advisor’s report.
4. Enter name of fire and date fire started, then circle whether you have or have not reviewed the official
State request for Fire Management Assistance Declaration under Section 420.
5. Enter the following prevailing weather conditions:
Temperature
Humidity
Wind
Other
6. Enter the type of fire index used (Palmer, KBDI, Haines, etc.) and enter the number on the scale.
7. Enter a description of the prediction of weather and fire conditions for the next 24 hours (fire behavior).
8. Wildfires:
8a. Describe the existence of other fires nearby that may result in a conflagration.
8b. Enter the number of large fires burning in the State.
9. Threat Information:
9a. Enter an assessment.
9b. Enter the proximity of fire to homes and communities.
9c. Enter the natural or other barriers between fire and communities.
10. Check the appropriate box, indicating whether the assessment is based on:
On site survey
Discussion with the Incident Commander
Other
11. Enter the signature of the Principal Advisor.
12. Enter phone numbers:
12a. Day phone
12b. Night phone
12c. Other (cell, pager, etc.)
13. Enter name of Federal Agency.
14. Enter Location.
