SSA uses Form SSA-783, the Statement Regarding Contributions, to collect information about one-half support and regular/substantial contributions the child applicant is currently receiving, and to evaluate their effect on the childÂs eligibility for Social Security benefits. The respondents are people who have information about the child applicantÂs current sources of support.
