Dear AARP Member,
We would like to ask you to participate in a new evaluation sponsored by the U.S. National
Institutes of Health (NIH) and AARP. The name of this study is iCLIC, which stands for
the Interactive Comprehensive Lifestyle Interview by Computer study.
Information from this study will add to our knowledge from an existing study called the
NIH-AARP Diet and Health Study. That study began in 1995, and more than 500,000
AARP members have participated. (See http://dietandhealth.cancer.gov for more
information.)
In iCLIC, we ask you to answer online questionnaires about your diet, lifestyle, and health
history over the past 24 hours and the past year. You will have 60 to 90 days to complete
up to four different questionnaires.
The goal of iCLIC is to evaluate a set of new online questionnaires that are replacing
the paper questionnaires you’ve completed in the past. As Internet communication gets
easier, researchers believe that online questionnaires will become a better way to gather
scientific information about health concerns.

We would greatly appreciate your participation in this study. To complete the questionnaires,
you will need a high-speed Internet connection and an email address. Please refer to the iCLIC
instruction card enclosed with this letter for full instructions. As explained on the card, the study
website can be found at www.webhealthstudy.gov. Use the Special Study Code on the front of
this letter to enter the site.
Once you have consented to participate in the study and answered some information about
yourself, you will create your username and password and be presented with your online
questionnaire schedule.
If you encounter problems registering or enrolling in the study, please send an email to
[email protected] or call toll-free to 1-888-302-6672 and we will get back to you as
soon as we can. Your participation is completely voluntary, and you may withdraw at any time.
